Our receptionist, Whitney, will guide you through the process of registering as a new patient to our office. She can answer general questions about our office or direct you to the appropriate person who can help you. She will set up your appointment and help you be prepared for your initial consultation.

What to prepare for your initial consultation:

• You will need to complete patient registration forms and return them at the time of your appointment.
• Request a copy of any previous medical records that are pertinent to your upcoming appointment. Have those records sent to our office prior to your appointment or bring them with you at the time of your appointment.
• Visit with your insurance company to determine your benefits
