Questions for Your Employer (Benefits Administrator)

• Are infertility treatments covered on my current health plan?
• Does the company offer another plan with fertility treatment? When I eligible to change plans?
• If there are no fertility treatment benefits, can I enroll in the company's flexible spending plan?

Contacting Your Employer or Insurer

Before calling your insurance company, HMO, or employer, have the following information at hand:

• Name of the insured individual
• Employee/Patient ID Number or Social Security Number
• Name of Employer
• Name of plan
• Group number/code
• Patient's name and date of birth

Be sure to get the name of the person with whom you are speaking, as well as his/her telephone and extension numbers.
