Preparing For Your First Visit

It is our pleasure to welcome you to Mid-Iowa Fertility. We are here to guide you through the process of diagnosis and treatment, starting with your initial consultation with your physician and nurse here at our clinic. The goal of this first visit is to thoroughly evaluate your medical history, outline a diagnostic plan to determine the specific needs of your case, and to design a treatment plan that is customized to your individual situation. The appointment typically lasts 45 to 60 minutes. During this visit, you will have the opportunity to have your questions answered and to participate in developing a plan for further evaluation and/or treatment. We strongly that encourage both partners attend this consult. In most cases, you will not complete any testing or have a physical exam during your initial consult. In other cases, if you’re on or near day 3 of your menstrual cycle, your physician may give you the option of initiating testing during this appointment or wait until the following month.

To make the most of your first visit, please plan to arrive a minimum of 15 minutes prior to your scheduled appointment time in order to complete any necessary paperwork.

Prior to your first appointment:

Because we all want your first visit to be the most productive we have created a new patient visit checklist to help you make the most of your first appointment. You will want to make sure your medical records are sent to our office before your first visit. In some cases, if records are not in your chart, it can delay your testing and/or treatment protocol.

Please bring the following with you to your appointment:

  • A copy of your medical records from your OB/GYN, primary care physician, or another fertility program. If you want medical records sent over to Mid-Iowa Fertility in advance, you must complete and send this release form to your physician. Note in some cases it may take 10 business days for your files to be sent over.

  • Completed Mid- Iowa Fertility new patient forms. Patient Demographic Form , HIPAA Form , Financial Policy Form , Insurance questionnaire Form

    (If you and your spouse/partner are coming to see us to achieve a pregnancy please both complete the following paperwork)

  • Insurance cards for both partners
  • A referral from your primary care physician, if applicable
  • Insurance authorizations; Champus/Tricare
  • Medical records from the urologist, if applicable
  • Hysterosalpingogram (HSG) report and film from the hospital or radiology laboratory, if applicable

During the initial consult, there is a lot to discuss; which can be overwhelming for some.  We encourage you to write down your questions in advance of your appointment and bring them with you to make the most of your time with your physician.

It is Mid Iowa Fertility’s policy to have chaperones in exam rooms only if requested by a patient. Please contact any of our medical staff with questions on this policy.

Commonly Asked Questions Regarding Your Initial Consultation with Mid Iowa Fertility

Please obtain copies of any pertinent medical records or imaging studies (e.g. hysterosalpingogram, MRI). Contact your referring physicians office to obtain these records. You can bring them with you for your first visit or ask that they be faxed to our office prior to your appointment (515-222-9563). If records are going to be faxed to our office, please call to confirm receipt prior to your visit.

Yes, we do accept insurance, however it will be your responsibility to contact your insurance provider and check your coverage options for fertility. Please bring this form with you at the time of your appointment.

Our office will mail you a new patient packet 1 week prior to your scheduled appointment.

Yes and we encourage that. Because this is a time for asking questions, we invite your partner to accompany you to your initial consultation to understand the process, get educated about procedures, ask questions and to support you. If your partner is not able to attend and you want a family member to come with you, that is fine too.

Please do not bring your children (or children under age 18) to your appointments as it can be insensitive to other patients struggling with fertility issues. If you are unable to obtain childcare please contact our office PRIOR to your appointment to see if there is any way we can help.

From when you arrive until you check out, expect to be in the office anywhere between 45 to 60 minutes. If you haven’t filled out your New Patient Forms before your appointment, arrive at least 30 minutes early to complete them. Otherwise, please arrive 10-15 minutes before your appointment.
Testing is based on different days of your menstrual cycle. Testing is usually done on day 3 of your cycle and days 6-13. Depending on where you are in your cycle it is possible that we could do some testing at your initial visit.
Because diagnoses vary significantly from patient to patient, consultation with your physician will establish the details and timing of your treatment plan, but in most cases treatment can often start within one cycle, approximately one month.
Because every patient’s situations are different, your doctor will determine timing and longevity of your plan.
